Welcome to the Paris Heart Centre The Heart Centre aims to improving the safety and quality of care through reporting, analysing and learning from adverse incidents and 'near misses' involving patients and staff. Central Paris & Paris Children’s University Hospitals NHS Trust has a duty to provide the highest achievable quality of care to its patients. As a large and complex organisation delivering a huge range of highly specialised services in a challenging environment, we must accept that risks are an inherent part of the day-to-day life of the Trust. The reporting of patient safety incidents and prevented incidents provides us with the opportunity to ensure that the learning gained from the experience in one area of the Heart Centre is used to reduce the risk of something similar happening elsewhere in the Heart Centre and indeed the Trust. The importance of feedback cannot be underestimated. To improve the current process of feeding back patient safety incidents a more organisational approach should be used to improve consistency (DOH 2000). Research has shown that the more incidents are reported, the more information is available about what is going wrong, and the more action can be taken to make healthcare safer. That is why it is important for all staff, both clinical and non-clinical, have the confidence and knowledge to report all patient safety incidents. (NPSA 2004) |
